                  This information is needed to assist us in locating a collection site.
                  Your annual MIS report requires this information. If you are a free lance Captain or deckhand it is NOT required.
                  An employee authorized by the employer to take immediate action(s) to remove employees from safety-sensitive duties, or cause employees to be removed from these covered duties, and to make required decisions in the testing and evaluation processes. The DER also receives test results and other communications for the employer. (This can be the Captain or even the spouse of a small operator)

                          Conditions of Membership

                When a member is selected for testing, they will be notified by one of the following methods: telephone, in person, US mail or certified mail. Once notification of the member has been made, they MUST IMMEDIATELY report for testing. Though not specified in the regulations, the following should be used as a general guiding policy: The marine employer must notify the member of the random drug test requirement, ONLY after the employer is sure that the collection site is available and the members schedule allows enough time for the specimen collection to take place. Once notified, the member must report immediately. No more than two to fours hours should lapse from the time of member notification to the time that the member reports to have a specimen collected. It is the recommendation of The Charterboat Consortium that ALL members appoint a Designated Employer Representative (DER) to receive test notifications. The DER is allowed to hold the test (without notifying the member) until such time as it is possible for them to report for testing (i.e. if the member is sick or on their days off, they would not notify them until they return to work.) In the case of a one person operation they could appoint their spouse or booking manager as the DER.

                Enrollment covers the cost of random, reasonable cause and post accident drug tests. Return to duty and follow up testing will be the responsibility of the member.

                Failure to take a RANDOM drug test, within the Coast Guard guidelines, constitutes a refusal to test and WILL be reported to the local US Coast Guard Investigations Office. The failure of ANY drug test (pre employment, random, reasonable suspicion or post accident) will also be reported to the US Coast Guard Investigations Office.

                Failure to abide by the above conditions will result in your removal from The Charterboat Consortium and forfeiture of all membership fees.
